Today's Field Brief shares updates and reminders for service providers reporting data for the State Opioid and Stimulant Response grant (SOS). The grant is funded by the Ohio Department of Behavioral Health (DBH) through the Substance Abuse and Mental Health Services Administration (SAMHSA) and evaluated by the Ohio Colleges of Medicine Government Resource Center (GRC).
SUPRT-A Reassessments for GPRA to SUPRT Data Transfers
SOS data is collected with SAMHSA’s Unified Performance Reporting Tools (SUPRT) and entered into the DBH iPortal application. If providers still need to submit a request to transfer GPRA to SUPRT, email SOS4@dbh.ohio.gov with the subject “SOS GPRA to SUPRT Data Migration”.
If you have paper SUPRT-A reassessments for clients with a GPRA intake that were collected within the six-month reassessment window but were not entered into iPortal on time, please email SOSEval@osumc.edu to request that the reassessment window be temporarily opened for data entry in the portal. DBH will adjust the baseline/intake date so the reassessment can be entered, and GRC will notify you when the portal is ready. Once data is entered, email GRC so that the original baseline/intake date can be restored. Please note: SUPRT-C reassessments are not yet available for entry in iPortal for clients who received a GPRA intake.
Data Collectors Must Complete SUPRT-C Record Management
The SUPRT-C Client or Caregiver Form is a voluntary, self-administered assessment for clients, proxies, and caregivers. While clients can choose not to complete the SUPRT-C, staff are required to complete the Record Management portion at the end of each SUPRT-C form each time a SUPRT-A is completed. The Record Management data for questions 1, 1a or 1b must be entered into the SOS iPortal. Staff must document the date of assessment, whether the client/caregiver completed the SUPRT-C, and, if not, the reason. Forms are available on the Documents | Ohio SOS Evaluation website.
New Guidance from SAMHSA on Clients Discharged During an Open Assessment Window
If a client is discharged during an open assessment window and program staff are still in contact with the client, staff should first offer the client or caregiver/proxy the opportunity to complete the SUPRT-C reassessment.
- If the client or caregiver/proxy completes SUPRT-C reassessment: Complete both a reassessment and closeout. Enter both SUPRT-A and -C reassessment data into iPortal first, then complete the SUPRT-A closeout.
- If the client or caregiver/proxy declines the SUPRT-C assessment: Do not complete a SUPRT-A reassessment or SUPRT-C reassessment. Complete the SUPRT-A closeout only.
Reminders: SUPRT-A Reassessment and Closeout Records
Actively Monitor Clients who Need Closeout Records
SOS service providers must actively monitor when clients end services and ensure that a SUPRT-A closeout record is completed and entered in iPortal. Using a resource like a client tracking spreadsheet can aid in data management by showing which clients are still active and need a closeout.
Complete Closeout if Client Is No Longer Engaged in Services: Reassessment Not Required After Closeout
- Following SAMHSA guidance, DBH is not collecting reassessment data after a closeout SUPRT-A is completed. When a client ends services and a closeout SUPRT-A is entered into iPortal before the reassessment window, then grantees should not complete the SUPRT-A reassessment or have the client complete the SUPRT-C reassessment.
- Client closeout and reassessment should never happen on the same day. If a client has ended services, a closeout should be completed, NOT a reassessment.
Complete Closeouts for Clients who Received a GPRA Intake when Services End
SOS service providers are required to use the SUPRT-A closeout for all clients no longer receiving services from the grant even if baseline and reassessment records were completed using the CSAT GPRA tool.

Sign Up for Training
How do I register for the new Awardee & Supervisor training?
All SOS-funded Alcohol, Drug Addiction, and Mental Health Services (ADAMHS) Boards, board sub-awardees, and agency service providers need to sign up for the new SOS Awardee and Supervisor Training. This one-hour training is designed for SOS-funded boards, grant managers, and supervisors who oversee SOS data collection, including SUPRT and quarterly reporting. Participants will review SOS data collection and reporting requirements and clarify key evaluation roles and responsibilities. Who needs to register for the required two-part training on SUPRT?
All data collectors, data entry staff, and managers who oversee data collection must complete the following two training courses:
- SUPRT Data Collection: Learning how to collect the SUPRT (90-minute presentation with optional 30-minute live Q&A)
- SUPRT Data Entry: How to enter the SUPRT into iPortal (one-hour presentation)
