SAMSHA Unified Performance and Reporting Tools (SUPRT) Data Collection

Effective October 1, 2025, SAMHSA has instructed grantees to replace the Government Performance and Results Act (GPRA) data collection tool with the SUPRT. Spanish versions of the tools will be forthcoming once released by SAMHSA.

SUPRT for Data Collection

SUPRT-Administrative (SUPRT-A)

The SUPRT-A is completed by grantees based on information available in client records. A SUPRT-A form needs to be completed for every client. Different sections are completed at baseline, reassessment, annual, and closeout.

SUPRT-Client (SUPRT-C) for Adults

The SUPRT-C for Adult is a set of brief questionnaires completed directly by the client. Different versions of the tool are used to collect client data based on when data is being collected.

SUPRT-Client (SUPRT-C) for Youth

The SUPRT-C for Youth is a set of brief questionnaires completed directly by the client. Different versions of the tool are used to collect client data based on when data is being collected. Only providers specifically funded to deliver SOS services to youth should collect data on clients under the age of 18.

SUPRT-Client (SUPRT-C) for Caregivers

The SUPRT-C for Caregivers is a set of brief questionnaires completed directly by the caregiver when a client cannot themselves. Different versions of the tool are used to collect client data based on when data is being collected.
