Today's Field Brief shares updates and reminders for service providers who collect and report data for the State Opioid and Stimulant Response grants (SOS 4.0).


Begin Informing Clients of Information Sharing and Collect Gift Card Data


Alert! Read the SOS Notice of Information Sharing to all Clients

DBH has provided a new Notice of Information Sharing and Background document that should be read to all clients. The notice is intended to let clients know that information will be shared with the program evaluators and will allow processing of gift cards. This incorporates and replaces the Program Overview Script. Read the notice with all new SOS 4.0 clients and any current clients still receiving services. Continue using SAMHSA SUPRT-C consent when offering the client the SUPRT-C assessment forms.

Prepare for Client Gift Cards

DBH has received approval from SAMHSA for $30 gift cards to be offered when clients complete the SUPRT-C 6-month reassessment and SUPRT-C annual assessment. Clients who are discharged from services before these reassessment periods are not eligible for the gift card. Gift cards can be physically mailed or electronically sent via text or email to the clients. Data Collectors must complete the Locator Form in iPortal with the client’s preferred method of receiving the gift card, along with updating the corresponding contact information.

Register for SUPRT Training

GRC continues to offer a two-part training series for SUPRT:

  • SUPRT Data Collection: Learning how to collect the SUPRT (90-minute presentation with optional 30-minute live Q&A)
  • SUPRT Data Entry: How to enter the SUPRT into iPortal (60-minute presentation)

Each session is offered live online twice per month. January and February dates are on the website. Register via the Training Events | Ohio SOS Evaluation page.


Plan for FFY26 SOS 4.0 Quarterly Reporting


Now Live! Submit SOS 4.0 Quarterly Report Data for Q1

SOS 4.0 Quarterly Reporting for Q1 opened on December 15, 2025, and will close January 12, 2026. All organizations directly funded by DBH are required to submit quarterly data. For Q1, this includes the SOR/TOR and TPPR.

Mark Your Calendars: SOS 4.0 Quarterly Reporting Dates

Table 1 displays the reporting schedule for the Federal Fiscal Year 2026 (FFY26) to allow providers to plan for upcoming reporting requests. Please note for FFY26, the Non-GPRA Tool is no longer fielded. If you have prevention programs, respond to the applicable questions in the SOR/TOR and PPR. GRC will host another live Question & Answer Session for Quarterly Reporting on January 8th.
