December 5, 2025 Field Brief

December 5th, 2025

*New* SUPRT

Today's Field Brief shares updates and reminders for service providers who collect and report data for the State Opioid and Stimulant Response grants (SOS 4.0).

Request Access to the SOS SUPRT iPortal App

As a reminder, the Ohio Department of Behavioral Health (DBH) has introduced a new portal for entering the SAMHSA Unified Performance Reporting Tool (SUPRT) data. All users must request access to the new SOS SUPRT application before entering data. Once approved, data can be entered into iPortal. For complete details, refer to this email communication from DBH.

Register for SUPRT Data Entry Training and Quarterly TA Sessions

Sign up for the New SOS SUPRT Data Entry Training

GRC is excited to introduce a new training: SUPRT Data Entry: How to enter assessments in iPortal. This training will help data collectors learn basic navigation tips with the new portal and go over how to enter SUPRT-A/C assessments. Training dates for December 19th and 23rd are now posted. Register on the Training Events | Ohio SOS Evaluation page. Weekly offerings of the SUPRT Data Collection training will continue to be offered through mid-December and will move to a twice-monthly cadence in January.

Join the SOS Quarterly Reporting Q&A Sessions

GRC will host live Question & Answer Sessions on December 18th and January 8th. The sessions are designed to provide an overview of the SOS Quarterly Reporting requirements and offer SOS-funded organizations an opportunity to ask questions about quarterly reporting. To register, please visit SOS Quarterly Reporting | Ohio SOS Evaluation and look for the registration links at the bottom of the page.

Important Update: Use both Patient ID and Client ID

SOS providers must now use a locally created Patient ID and DBH iPortal-generated Client ID. When entering SUPRT-A forms into the DBH portal, SOS providers will need to:

  • Create a unique Patient ID locally and use it to start a SUPRT-A baseline in the DBH iPortal. The Patient ID must be non-identifiable. Do not use any part of the following:
    • No part of a client’s date of birth.
    • No part of a client’s Social Security Number.
    • No part of a client’s Medicaid ID
    • No part of a client’s name.
  • After submission of SUPRT-A baseline, the portal will auto-generate a Client ID.
    • Immediately record the Client ID on corresponding paper SUPRT forms.
    • Immediately record Client ID on your local client tracking spreadsheet.
    • Use the same Client ID for SUPRT-A and SUPRT-C forms and across all episodes of care for the client.
