 Today's Field Brief shares updates and reminders for service providers reporting data for the State Opioid and Stimulant Response grant (SOS). The grant is funded by the Ohio Department of Behavioral Health (DBH) through the Substance Abuse and Mental Health Services Administration (SAMHSA) and evaluated by the Ohio Colleges of Medicine Government Resource Center (GRC). Data is collected with SAMHSA’s Unified Performance Reporting Tools (SUPRT) and entered in the DBH iPortal application.


SOS Q3 Reporting Now Open - Submit by July 13


Quarter 3 reporting is now open and will close on July 13th (Monday) at 5:00 pm. All organizations directly funded by DBH are required to submit quarterly data. Please see the list of SOS 4.0 Year 2 awardee reporting requirements (last updated May 1, 2026). If you do not see your organization’s name on the list, please email SOS4eval@dbh.ohio.gov and CC SOSEval@osumc.edu to confirm your reporting responsibilities. Q3 reporting includes the SOR/TOR Program Instrument which has a reporting period of April 1, 2026 – June 30, 2026. If your organization was awarded supplemental funding, you must submit a separate SOR/TOR.

Biweekly Reports Have Resumed Delivery


Biweekly reporting was reinstated on June 22nd. The GRC sends biweekly reports to provide timely program data and help guide your work. Direct awardees will receive a biweekly report email from SOSEval@osumc.edu every other week. Additional resources including a walkthrough tutorial, delivery dates, and a technical assistance form can be accessed on the SOS 4.0 Biweekly Reporting page.


Updates in iPortal


DBH has released multiple updates to the SOS SUPRT iPortal application. They include the following:

  • IMPORTANT! Collectors are now able to enter SUPRT-C reassessments (6-month and annual) for clients that started SOS services with a GPRA baseline. If you have paper copies of SUPRT-C 6-month and annual assessments for your clients with a GPRA baseline, you may now go into the portal and enter those records. If the window for entering the record has closed, please reach out to SOSEval@osumc.edu so GRC can assist you. This process will require multiple steps:
    1. Email us with the subject line: “SUPRT-Cs for GPRA clients”
    2. Provide a full list of the clients that require reopening of their reassessment using the table provided below. GRC will need to know if the SUPRT-C is a 6-month or annual assessment (or both) and which date the client completed the SUPRT-C reassessment on paper.
    3. GRC will submit a ticket to get the reassessments reopened. When they are reopened, GRC will let you know.
    4. Enter the reassessments as soon as possible upon reopening.
    5. Email GRC when your records have been entered. Once complete, GRC will ensure the baseline and reassessment dates are correct with DBH.

  • All data for clients with a GPRA baseline that was migrated to the new SOS SUPRT portal will now have “Migrated” tag on it. This will allow collectors to easily identify which clients data was migrated over from the previous SOS portal. Data shown below is fake and not reflective of real client data.

  • Based on new SAMHSA guidance, if a client is discharged during an open assessment window and program staff are still in contact with the client, staff should first offer the client or caregiver/proxy the opportunity to complete the SUPRT-C reassessment. If you attempt a closeout during the open-window for reassessment, you will receive a warning to let you know that the client is eligible for completing their SUPRT-C before you proceed to closeout.

Gift Cards Are Now Processing


On May 26, gift card processing resumed for eligible SOS 4.0 clients. Agencies should keep using the paper Gift Card Order Form to collect the client’s contact information and delivery preferences. Then, submit the request through the Locator Form in iPortal.

As a reminder, eligible participants may receive a $30 gift card for completing a SUPRT-C 6-month reassessment or a 12-month annual assessment. Forms must be collected within a valid data collection window, which opens 30 days before the assessment due date and closes 30 days after. Participants who receive a closeout assessment prior to the reassessment are not eligible to receive a gift card.

  • SUPRT Data Collection: Learning how to collect the SUPRT (90-minute presentation with optional 30-minute live Q&A)
  • SUPRT Data Entry: How to enter the SUPRT into iPortal (one-hour presentation)    
     

Register for the Awardees and Supervisor Training

Registration is still open for the one-hour SOS Awardee and Supervisor Training. SOS-funded boards, sub-awardees, and service providers should attend to review SOS data collection and reporting requirements (including SUPRT and quarterly reporting) and clarify evaluation roles and responsibilities.
