Today's Field Brief shares updates and reminders for service providers reporting data for the State Opioid and Stimulant Response grant (SOS). The grant is funded by the Ohio Department of Behavioral Health (DBH) through the Substance Abuse and Mental Health Services Administration (SAMHSA) and evaluated by the Ohio Colleges of Medicine Government Resource Center (GRC). Data is collected with SAMHSA’s Unified Performance Reporting Tools (SUPRT) and entered in the DBH iPortal application.
Submit Requests for GPRA Data Migration
All SOS service providers need to make sure they have worked with DBH to migrate SOS 4 Year 1 clients to the new SUPRT portal. If you have not already done so, send an email to SOS4@dbh.ohio.gov with the subject “SOS GPRA to SUPRT Data Migration.” DBH will send a form that needs to be returned to them that will indicate the current active collectors.
Data Management Best Practices
Ensure All SOS Clients have SUPRT-C Records Entered
Missing SUPRT-C records will result in failed assessments in iPortal. The Record Management portion of SUPRT-C is always required at baseline, and, as applicable, at 6-month and annual timepoints, regardless of whether a client has completed the form (see image below).

Key Reminders
- Client participation in SUPRT-C is voluntary; however, staff must always complete and enter the required Record Management questions in iPortal for every timepoint. On the paper record, this section is at the very end of each form. Every client must have both a SUPRT‑A and a SUPRT‑C record entered in iPortal.
- Catch up on any SUPRT-C records you have missed! These records can be entered into iPortal at any time.
- Please check iPortal and your client tracking spreadsheet to see if all clients have both a SUPRT-A and SUPRT-C record entered. If needed, data collection forms are available on the Documents | Ohio SOS Evaluation page.

Monitor and Complete Administrative Closeouts When SOS Services End
When a client completes or stops receiving SOS-funded services, staff must collect an Administrative Closeout Record. This is done by collecting Section A and E of the SUPRT-A Form. This requirement applies to all SOS 4 clients, including those with GPRA baselines.
See below for ways to identify clients without a completed closeout record:
- Review your client tracking spreadsheet.
- Check all clients listed under the “Eligible Admin Record Closeouts” button on the iPortal menu bar.
- Download and review a client list from iPortal by selecting “Search Admin Assessments,” choosing your care provider/agency, and clicking “Export Search Results to Excel.” Once downloaded, review the “Assessment Type” to identify clients who do not have a Record Closeout.

How to Submit Effective TA Requests
Submit clear, detailed requests for technical assistance (TA) to help the GRC Evaluation Team be able to respond quickly. Use the guidance below when requesting support.
Details to Include
- Full organization name (no acronyms)
- Clear description of the issue
- Client ID and Patient ID (if applicable)
- Assessment type and date (if applicable)
- Names of collectors (if submitting record transfer requests)
- Screenshots (if having iPortal issues)
Best Practices
- Be as specific as possible to help speed up review and response
- Never include client name or date of birth
- Double-check that screenshots and messages do not include identifying information
- Ensure that you have completed both required training courses: SUPRT Data Collection and SUPRT Data Entry

Mark Your Calendars
SOS June Quarterly Call will be held on June 8 and June 9
DBH will be hosting the June Quarterly Monitoring Call on June 8 and June 9. Awardees received a save the date email with a link to join the call on either day. The call will cover topics regarding GFMS, updated monitoring processes, and reporting information. The agenda is the same for both days, so plan to attend at least one of the call times listed below.

SOS Quarter 3 Report due July 13
Thank you to everyone who submitted their Quarter 2 reports in April. It is never too early to start preparing for the next upcoming quarterly reporting period. Quarter 3 will include reporting on the SOR/TOR instrument for the period of April 1, 2026 to June 30, 2026.
- A preview email will go out to direct awardees on June 8 with more details.
- If your organization is funded by a local board or another entity that is not DBH, reach out to your project lead for further instructions.
- Quarter 3 reporting will open on June 15 and will close on July 13 at 5:00 PM. Late submissions will not be accepted.
- Live Question and Answer sessions are scheduled for June 16 at 1:30 PM and July 9 at 10:00 AM.

- SUPRT Data Collection: Learning how to collect the SUPRT (90-minute presentation with optional 30-minute live Q&A)
- SUPRT Data Entry: How to enter the SUPRT into iPortal (one-hour presentation)
Register for the Awardees and Supervisor Training
All SOS‑funded behavioral health boards, sub‑awardees of boards, and agency service providers who have not yet registered should sign up today for the one‑hour SOS Awardee and Supervisor Training. The training reviews SOS data collection and reporting requirements, including SUPRT and quarterly reporting, and clarifies evaluation roles and responsibilities.
