The Field Brief today shares updates and reminders for service providers who collect and report data for the State Opioid and Stimulant Response grants (SOS 3.0 and 4.0).
Reporting
The end of year PPR and Quarter 4 SOR/TOR reporting forms are now available (look for reporting resources here: SOS Quarterly Reporting | Ohio SOS Evaluation). Each organization directly funded by the Ohio Department of Behavioral Health (DBH) is required to complete both the PPR and SOR/TOR reports. For the SOR/TOR, reporting is open for Quarter 4 only (July 1, 2025 through September 29, 2025). The reporting timeframe for the End-of-Year PPR is the full grant period from September 30, 2024 through September 29, 2025. The deadline to submit responses is Monday, October 13, 2025.
Use SUPRT for all client-level data collection.
Data Collection
Copies of the new SAMHSA Unified Performance and Reporting Tools (SUPRT) are available on the Ohio SOS Evaluation Resources webpage. The SUPRT-A (Administrative) and SUPRT-C (Client) have replaced the GPRA tool for all client-level data collection and should now be used for all active and new SOS 4.0 clients. This overview comparison resource includes details on changes between the GPRA and SUPRT tools.
- SUPRT-A is completed by agency staff using information from client records, rather than interviews with the client. SUPRT-A is mandatory for all clients.
- SUPRT-C is self-administered by clients, caregivers, or proxies. Client participation in SUPRT-C is voluntary.
How do we transition clients with a GPRA to SUPRT? It depends on whether the client is still receiving SOS 4.0 services. If an SOS 4.0 client with a GPRA baseline record is:
- Still receiving services, collect the 6-month SUPRT-A reassessment and annual SUPRT-A when due. These clients will not be required to complete the SUPRT-C forms (Note: this only applies to clients with a GPRA baseline. This requirement may change for clients with a SUPRT-A baseline assessment after October 1, 2025).
- No longer receiving SOS 4.0-funded services, you are required to use the SUPRT-A closeout, even if the baseline and 6-month reassessment records were completed using the GPRA tool.
Complete the SUPRT-A and SUPRT-C forms on paper for online data entry when the new SOS iPortal launches. For new SOS 4.0 clients, use the SUPRT-A and SUPRT-C for assessments. 1. Add the SOSEval@osumc.edu email address to your contacts.
2. Bookmark and check the SOS evaluation website: OhioSOSevaluation.org.
3. Share the sign-up form for the news brief to stay up to date on SOS news.
