October 24, 2025 Field Brief

October 24th, 2025

*New* SUPRT

This week's Field Brief shares updates and reminders for service providers who collect and report data for the State Opioid and Stimulant Response grants (SOS 4.0).

iPortal Access

A new portal is being developed for entering the SAMHSA Unified Performance Reporting Tool (SUPRT) data, which will be hosted by Ohio Department of Behavioral Health (DBH). Once the new data entry portal is available, details will be shared.

Create and maintain a list of SOS clients

SOS Client List

Agencies need to keep a separate list of clients who receive SOS services, maintained outside of the portal, as a strong data management practice. An updated client tracking template is now available for download from the SOS evaluation website. It includes the new SUPRT-A assessments and includes space to:

  • Maintain a list all clients who receive SOS services. This helps staff track key identifies such as client names and iPortal patient ID.
  • Monitor due dates and upcoming assessments using columns that calculate open windows from the date of baseline. Note: SUPRT windows for reassessment and annual assessment are now calculated in days, unlike GPRA which used months.
  • Ensure the closeout assessment is completed when a client’s episode of care ends.
  • Cross-check data entry and maintain records for potential audits.

SUPRT-A and SUPRT-C assessment forms will be entered into iPortal for centralized reporting once it is back online.

SUPRT Training

If you have not already done so, visit the Training Events | Ohio SOS Evaluation page to register for SUPRT training. In November, dates are offered on a weekly basis for SUPRT Data Collection: Learning how to collect the SUPRT. The same content is covered each week. A companion training on SUPRT Data Entry will be available after the new iPortal launches. Those training dates will be posted on the SOS evaluation website.

Data Collection and FAQs

Updated details about the new SAMHSA Unified Performance Reporting Tools (SUPRT) have been posted on the Ohio SOS Evaluation Home Page. Information includes specifics on how SUPRT-Administrative (A) and SUPRT-Client (C) have replaced the GPRA tool for all client-level data collection. The FAQs | Ohio SOS Evaluation page contains answers to common questions about SUPRT data collection, including how to transition existing SOS 4.0 clients who have a GPRA to the SUPRT.
