Today's Field Brief shares updates and reminders for service providers who collect and report data for the State Opioid and Stimulant Response grants (SOS 4.0).
Reporting
The end of year PPR and Quarter 4 SOR/TOR reporting forms are now available (resources for reporting can be found here: SOS Quarterly Reporting | Ohio SOS Evaluation). Each organization directly funded by the Ohio Department of Behavioral Health (DBH) is required to complete both the PPR and SOR/TOR reports. The deadline to submit responses is Monday, October 13, 2025.
SUPRT now replaces the GPRA Tool for all client-level data collection.
Data Collection
Look for copies of the new SAMHSA Unified Performance and Reporting Tools (SUPRT) on the Ohio SOS Evaluation Resources webpage. The SUPRT-A and SUPRT-C have replaced the GPRA tool for all client-level data collection. This overview comparison resource includes details on changes between the GPRA and SUPRT tools, including that there are now two separate data collection tools, SUPRT-A and SUPRT-C.
- SUPRT-A is completed by agency staff using information from client records, rather than interviews with the client. Different sections of SUPRT-A are completed at the client’s baseline, reassessment, annual review, and closeout. This schedule differs from the GPRA. While some questions in SUPRT-A questions resemble those in the GPRA, wording and answer choices often vary.
- SUPRT-C is completed by the client. It is self-administered or completed by a caregiver, instead of having the client answer questions in an interview with a data collector. Staff will also need to access their agency records or clinical documentation to finish certain parts of the assessment. Different versions of the tool are used depending on when the data is being collected, at the client’s baseline, reassessment, and annual review.
Additionally, SAMHSA has released new guidance to support implementation of these new tools, including the SUPRT-A Question-by-Question (QxQ) Guide and the SUPRT-C QxQ Guide. One note: these documents reference the SAMHSA Performance Accountability and Reporting System (SPARS). The DBH (formerly OhioMHAS) handles all interfaces with SPARS for Ohio awardees.
