Today's Field Brief shares updates and reminders for service providers reporting data for the State Opioid and Stimulant Response grant (SOS). The grant is funded by the Ohio Department of Behavioral Health (DBH) through the Substance Abuse and Mental Health Services Administration (SAMHSA) and evaluated by the Ohio Colleges of Medicine Government Resource Center (GRC). Data is collected with SAMHSA’s Unified Performance Reporting Tools (SUPRT) and entered into the DBH iPortal application. If providers still need to submit a request to transfer GPRA to SUPRT, email SOS4@dbh.ohio.gov with the subject “SOS GPRA to SUPRT Data Migration”. In addition, quarterly reporting tools are used to collect required federal data that are submitted through a REDCap link emailed to grant awardees.


Begin using the new Gift Card Order Form and entering Annual Assessments

In partnership with DBH, GRC has developed a new Gift Card Order Form to support distribution of the $30 gift cards for clients who complete a SUPRT-C 6-month reassessment or SUPRT-C 12-month annual assessment. The Gift Card Order Form is now available to download from the SOS website. Providers should use the new Gift Card Order Form with clients who are still receiving services and are completing their SUPRT-C 6-month or SUPRT-C 12-month assessments. Providers should continue to use the Notice of Information Sharing and Background with clients during the baseline assessment.

The new client-facing Gift Card Order Form includes an overview of SUPRT-C data collection, gift card eligibility and details, and client consent to share their information. The form also allows the client to select how they prefer to receive their gift card – either by mail or electronically sent via text or email. Clients will need to provide a valid mailing address, mobile phone number, or email address based on their preferred method of delivery. Once signed, the Gift Card Order Form must be retained locally at the provider agency in accordance with agency record-retention policies.

DBH is currently updating the iPortal system to replace the Locator Form prompt with the Gift Card Order Form. Once this update is complete, only the new Gift Card Order Form will be prompted, and it will only appear during the SUPRT-C 6-month reassessment and the SUPRT-C 12-month assessment. Data collectors should continue entering locator form and gift card information for clients in the iPortal SUPRT application at all assessment timepoints and collect the Gift Card Order Form on paper at relevant timepoints.

Enter Annual Assessments

DBH has released a new function in the iPortal SOS SUPRT app to allow Annual Assessments to now be entered for both SUPRT-A and SUPRT-C. On the dashboard page of the SUPRT app:

  • Look for a new button in the top banner that says “Annual Assessments” where collectors will find eligible annual assessments.
  • In the dropdown, click “SUPRT-A Annual Assessments” to enter the administrative record and click “SUPRT-C Annual Assessments” to enter the client record.

Records that are in the 60-day open window of their 12-month anniversary will appear in the list on these tabs for data entry. Annual assessments are only conducted with clients that are still receiving services. If a client’s record is closed out prior to their one-year anniversary, they will not receive an annual assessment.

As a reminder, GRC always recommends the best practice of collecting all client assessments on paper forms and then entering the data afterwards into the portal. This allows for better record keeping and avoids potential data loss. SUPRT-A/C paper records can be found on the SOS Evaluation website on the Resources Page. With annual assessments now available for entry, please make sure to enter any that have been collected on paper into the portal.

GRC will offer additional sessions for the new SOS Awardee and Supervisor Training on February 27th, March 6th, March 20th, and April 14th. This one-hour training is designed for all SOS grant managers and supervisors who oversee SUPRT data collectors and quarterly reporting. The session covers SOS data collection, reporting requirements, and key evaluation responsibilities. GRC has also added dates for the required two-part training series on SUPRT. All data collectors, data entry staff, and managers overseeing data collection must complete both courses:

  • SUPRT Data Collection: Learning how to collect the SUPRT (90-minute presentation with optional 30-minute live Q&A)
  • SUPRT Data Entry: How to enter the SUPRT into iPortal (one-hour presentation)

Review list of grantees required to submit Quarterly Reports


Mark your calendars for SOS 4.0 Quarter 2 Reporting

Quarter 2 reporting will open on March 16th and close April 10th (Friday). All organizations directly funded by DBH are required to submit quarterly data. Please see the list of SOS 4.0 awardee reporting requirements. All SOS awardees are required to collect and submit quarterly SOR/TOR and bi-annual PPR reports. SOS 3.0 NCE grant awardees must also submit a separate SOR/TOR Sub-Recipient Report for that grant cycle. For Q2, reporting includes the SOR/TOR, SOR/TOR Sub-Recipient Reporting, and Mid-Year PPR. Please note: these downloadable tools are for reference and preparation only. All final responses must be submitted through the emailed REDCap link. As a reminder for FFY26, the Non-GPRA Tool is no longer fielded. If you have prevention programs, respond to the applicable questions in the SOR/TOR and Performance Progress Report (PPR).
