Today's Field Brief shares updates and reminders for service providers reporting data for the State Opioid and Stimulant Response grant (SOS). The grant is funded by the Ohio Department of Behavioral Health (DBH) through the Substance Abuse and Mental Health Services Administration (SAMHSA) and evaluated by the Ohio Colleges of Medicine Government Resource Center (GRC). Data is collected with SAMHSA’s Unified Performance Reporting Tools (SUPRT) and entered in the DBH iPortal application.


Enter All Paper SUPRT Forms into iPortal


Please enter any held paper SUPRT forms into iPortal as soon as possible. If your organization has any paper forms that have not yet been entered, please prioritize entering them. Paper forms should be entered into iPortal within one week of data collection to help ensure timely and complete records.

SUPRT-C Reassessment Data Entry

The SOS SUPRT iPortal application has been modified to allow entry of all SUPRT-C reassessments for clients with GPRA baselines. Please make sure all SUPRT-C 6-month and annual assessments are entered in iPortal.

If the window for entering the reassessment has closed, reach out to the GRC SOS Evaluation Team for assistance by sending an email to SOSEval@osumc.edu. We will need to know the Client ID, Patient ID, GPRA Baseline Date, Type of SUPRT-C Assessment, and Date the Reassessment was completed by the client. Gift Card Processing Reminders

Gift card processing began in late May. Agencies should continue using the paper Gift Card Order Form to collect client contact information and delivery preferences. Then, submit the gift card request through iPortal. Paper copies should be kept locally according to your organization’s record retention policies. Do not send paper forms to GRC. Complete SUPRT-C Forms at Baseline

In iPortal, all clients must have a SUPRT-C record at baseline. Even if a client declines participation in the SUPRT-C, collectors are required to fill out the record management section of the assessment and enter this information into the portal. There is a new section on the iPortal dashboard called “Client Missing Baseline – Action Required.” Records that are missing a SUPRT-C assessment at baseline can be found here. Collectors need to check this section regularly and complete any records found there before entering any subsequent 6-month or annual assessments.


Gift Card Order Form Replaces Locator Form in iPortal: Updates in Progress

Updates to align iPortal with the SOS SUPRT-C gift card process are currently being implemented. Part of these changes involve the SOS Gift Card Order Form replacing the Client Locator Form in iPortal. Clients remain eligible for a $30 gift card at the SUPRT-C 6-month reassessment and annual assessment timepoints if they are still receiving services at those timepoints. No gift card is available at baseline.

The Gift Card Order Form must be completed by clients on paper, and clients must provide consent to share their contact information with GRC. After obtaining consent, collectors should enter the client’s contact information and preferred delivery method in iPortal. Gift cards may be delivered by mail, text message, or email. Please verify that all information is accurate before submitting to help ensure successful delivery. The Gift Card Order Form can be accessed from the Patient Assessment History page in iPortal.

It is recommended that sites continue to collect the locator form locally to support client outreach. Both the SAMHSA Locator Form and the SAMHSA Mini Locator Form are available on the SOS Evaluation website for download and use.

Please note: The gift card section of iPortal will continue to be enhanced over the coming weeks to more fully reflect the Gift Card Order Form process.


Check Out the SOS Evaluation Website Updates


The SOS Evaluation website has been updated to make it easier to find commonly used forms, guidance, and resources. These changes do not affect forms or data collection requirements; they only reorganize existing resources. Below is a summary of key enhancements:

  • Home Page: Updated to better highlight SOS data collection requirements, including Quarterly Reporting and SUPRT Data Collection. Added quick links to frequently used forms and resources, such as SUPRT-A/C forms, the Notice of Information Sharing, and the Gift Card Order Form.
  • Documents Page: Organized into clearer categories with simplified file names to make materials easier to locate. Added a new Key Resources section featuring the adult SUPRT forms, client engagement materials, and data entry resources.

These updates are intended to reduce time spent searching for information and support timely, accurate data collection and entry. Additional website enhancements will be announced in future communications.


Thank You for Completing Q3 Reporting


Thank you to everyone who submitted their Quarter 3 reports in July. It is never too early to start preparing for the next upcoming quarterly reporting period.

Mark your calendars! Quarter 4 will include reporting on the SOR/TOR Instrument for the period of July 1, 2026 to September 29, 2026 and the End of Year Performance Progress Report (PPR) for the period of September 30, 2025, to September 29, 2026.

  • A preview email will go out to direct awardees on September 7 with more details.
  • If your organization is funded by a local board or another entity that is not DBH, reach out to your project lead for further instructions.
  • Quarter 4 reporting will open on September 14 and will close on October 12 at 5:00 PM. Late submissions will not be accepted.

Updates on Training


Look for August Training Dates

August training dates are now available. Please visit the Training Events | Ohio SOS Evaluation website to register.

  • All data collectors, data entry staff, and managers who oversee data collection must complete the annual two-part training series: SUPRT Data Collection and SUPRT Data Entry. Because these live trainings are offered separately, staff must register for each session individually.
  • The Awardees and Supervisor Training is another option available. The training reviews SOS data collection and reporting requirements, including SUPRT and quarterly reporting, and clarifies evaluation roles and responsibilities. 
     

Coming Soon! New Training Platform: Canvas

The SOS Evaluation Team is expanding training opportunities for SOS grantees to support continued learning and engagement. In addition to accessing the live trainings offered by the GRC team, a series of self-paced learning modules will be available soon on the Canvas platform. Stay tuned for additional details and upcoming announcements about these new training resources and opportunities.
