All SOS 3.0 & 4.0 GPRA data collectors should complete training on GPRA data collection, and any staff entering GPRA data into the SOR SOS iPortal application should be trained on iPortal data entry. SAMHSA requires that GPRA intake interviews be completed within 3 days of the client beginning SOS-funded residential treatment or services and 4 days of the client beginning SOS-funded outpatient treatment or services. SAMHSA requires that GPRA interviews are to be submitted into iPortal within 7 days of completion.
Step 1. Gather necessary information
- SOS Consent
- Locator form
- Agency name, funding type, and unique iPortal client ID
- CSAT GPRA Form
- Planned services for the client
Step 2. Ensure eligibility and complete consent
Step 3. Fill out locator form to ensure that client can be reached for follow-up
- Fill out locator form to ensure that future outreach for follow-up
- Make sure the client gives three other contacts that can be reached
Step 4. Administer CSAT GPRA intake interview
- Complete sections A-G of the CSAT GPRA tool
- Use the SAMHSA QxQ and FAQs documents to guide administration of the interview
Step 5. Upload GPRA and locator form data to OhioMHAS iPortal SOR SOS Application
- Log in to the iPortal SOR SOS Application with your OH|ID (How-to guide)
- Enter Client ID
- Enter agency name, funding type
- Confirm consent
- Enter GPRA data
- Enter locator form data
